Clean up some logic and changelog

We want to preserve functionality people may be relying on (i.e. apps without active_record were accidentally getting a database.yml if they had a config/ directory. We can re-visit this later, but for now want to preserve backwards compat.

Remove duplicate test already tested in rails4_spec.rb
